The easiest way (in my opinion) to make a cursor is to first start out by downloading a set of cursors.

I downloaded this set for this tutorial.

Next head to your home folder, and hit CTRL+H to show the hidden folders, and go to the .icons folder.

Now open up the tarball you've just downloaded and move the folder (I moved the blue one) into the .icons folder. Now if there isn't a folder, then go ahead, and make one.

Now go head and head to the source folder, edit the images or make a whole new set of cursors in there, and apply the same to appspinner, and the spinner folders. Then you can go ahead and edit the configure files in there, but I myself I'd let them be, and my last bet would be to rename the folder from "PolarCursorTheme-Blue" so something else like "Glossy-Blue" or w/e you'd like.
